No, mistakes don't "happen", she MADE a stupid mistake because she wasn't fit to be in space. She was only on that shuttle because she was FEMALE - how many more qualified men do you think missed their chance because the assholes at NASA wanted to be more 'diverse'? A thousand? Ten thousand? More than that, I bet.
Her actions, clearly caught on video, show that she is not fit to do the job, and look at how much it's cost us, the taxpayers - $100,000.
